The iPhone 12 mini and iPhone 12 Pro Max are not affected by this issue.

Apple itself does not provide any tool through which you can know if your iPhone 12 or iPhone 12 Pro unit is impacted or not. Nonetheless, if you are facing sound issues with the earpiece on your unit, it’s likely eligible for the service recall.

The service program covers affected iPhone 12 and iPhone 12 Pro units for two years from the first sold date. So, if your iPhone starts exhibiting issues at a later date, you can still get it repaired for free from Apple.

The program is also applicable worldwide, so you can get your iPhone 12 repaired for free, irrespective of your location. However, Apple may limit the repair to the original country of purchase.
